The Women's Health Lab Edit

The changing female body deserves more than a hormone snapshot. Begin with cycle history, nourishment, sleep, symptoms, and the daily conditions shaping your health.

By Dr. Michele Burklund, ND · Updated November 1, 2026

Begin with the foundations

A question before a panel.

Begin with the question, then choose the test. Food, sleep, movement, connection, oral care, and relevant environmental changes remain the foundation. Laboratory work is most useful when a finding could change a decision.

01 / THYROID & NOURISHMENT

Thyroid & Nourishment

Discuss thyroid assessment, CBC, ferritin, or selected nutrient tests when history and symptoms justify them. Blood tests should answer a specific question about fatigue, bleeding, intake, or absorption; they are not a reason to order every available nutrient panel.

02 / CYCLE & HORMONES

Cycle & Hormones

Timed reproductive hormones may clarify selected questions. In otherwise healthy people aged 45 or older with typical symptoms, perimenopause is usually identified clinically. A single estrogen or FSH result can miss cycle-to-cycle variation.

03 / METABOLIC HEALTH

Metabolic Health

Glucose assessment and cardiovascular risk review can be more useful than repeatedly measuring fluctuating sex hormones. A1c reflects longer-term glycemia but may be misleading with anemia or altered red-cell turnover.

04 / FEMININE ECOLOGY

Feminine Ecology

Vaginal testing may be considered for persistent or recurrent symptoms. Pair findings with tissue health, symptoms, examination, and a review of recent products or antimicrobials.

A closer look

What the findings can tell us.

01

Comprehensive hormone profile

Dried-urine testing explores sex-hormone levels, the metabolites produced as those hormones are processed, and cortisol patterns across the day. I consider this profile when looking more closely at menstrual changes, midlife symptoms, sleep, and stress physiology. The results belong within the story of your cycle, symptoms, and hormone use; follow the collection instructions for timing.

Specimen
Timed samples using the exact selected hormone kit.

Timing & collection
Record sleep timing, cycle day when relevant, hormone use, and collection times. Do not substitute DUTCH Complete, Plus, or Cycle Mapping schedules for one another.

Daily patterns are affected by timing, medicines, and collection conditions. These tests do not diagnose grief or adrenal fatigue.

02

Comprehensive stool analysis

A stool-based look at digestive function, intestinal inflammation, and selected organisms within the gut microbiome. This can add detail when I am investigating persistent digestive symptoms or looking at how well the foundations of nourishment are working. I consider the findings alongside your food intake, bowel patterns, medication history, and symptoms when deciding which questions to explore next.

Specimen
Stool collected with the selected kit.

Timing & collection
Follow the selected laboratory instructions for collection days, sample handling, and recent antimicrobial or supplement use.

A stool result is a snapshot. Microbial associations do not prove a cause of symptoms or establish one ideal microbiome; individual markers differ in clinical validation.

03

Comprehensive metabolic panel

A foundational blood panel that measures glucose, electrolytes, calcium, proteins, and markers related to liver and kidney function. It brings several connected systems into view at once. In my approach, these results help establish context before adding more specialized testing, and I look at patterns and changes over time alongside hydration, nutrition, and your health history.

Specimen
Blood; confirm the exact specimen and collection requirements on your order.

Timing & collection
Follow the instructions supplied with the exact order. Fasting and medication timing may change when tests are combined.

Interpret with the reason for testing, symptoms, medical history, and the laboratory method. An isolated result is not a diagnosis.

04

Iron studies & ferritin

Looks at iron circulating in your blood, its transport and binding capacity, and the iron held in reserve as ferritin. I consider the full pattern when exploring fatigue, heavy menstrual bleeding, or possible nutritional depletion. These measurements help distinguish different iron patterns and inform a more thoughtful conversation about food, absorption, blood loss, and whether iron supplementation is appropriate.

05

Thyroid screening

Begins with thyroid-stimulating hormone, the signal that helps regulate thyroid hormone production. Free T4 is added automatically when the TSH result meets the laboratory’s criteria. This is a starting point for exploring thyroid function in the context of energy, temperature tolerance, bowel patterns, or menstrual changes; it does not automatically include thyroid antibodies or every thyroid measurement.

06

Apolipoprotein B

Measures apolipoprotein B, a protein found on cholesterol-carrying particles that can contribute to plaque in the arteries. It adds information about particle burden beyond the amount of cholesterol reported on a standard lipid panel. I consider it alongside blood pressure, glucose, family history, nutrition, and movement as part of a broader cardiovascular and longevity assessment.

Blood; confirm the exact specimen and collection requirements on your order.

Follow the instructions supplied with the exact order. Fasting and medication timing may change when tests are combined.

Interpret with the reason for testing, symptoms, medical history, and the laboratory method. An isolated result is not a diagnosis.

Hemoglobin A1c & estimated average glucose

Measures the proportion of hemoglobin with glucose attached, providing a view of average blood sugar over approximately two to three months. Estimated average glucose translates that result into familiar blood-sugar units. I use the longer view to explore metabolic patterns alongside nourishment, movement, sleep, and other relevant markers; it does not show individual meal-related spikes.

Specimen
Blood; confirm the exact specimen and collection requirements on your order.

Timing & collection
A1c alone does not require fasting; other tests in the same order may.

Anemia, hemoglobin variants, recent blood loss or transfusion, and altered red-cell turnover can affect interpretation. It does not reveal individual meal-related glucose peaks.

08

Vaginal microbiome profile

Examines microorganisms present in a vaginal sample, offering a more detailed view of the intimate microbiome. I consider this information when exploring recurring changes in discharge, odor, irritation, or vaginal comfort. The findings are interpreted alongside symptoms, hormonal changes, and treatment history; the presence of an organism alone does not tell the whole story.

Specimen
Vaginal swab.

Timing & collection
Collect outside menstruation and follow kit-specific restrictions around vaginal products, sex, and recent antimicrobials. Arrange medication decisions with the clinician.

Interpret microbial detection with symptoms and examination. This is not a urine culture or a complete explanation for recurrent urinary symptoms.

09

Anti-Müllerian hormone

A blood measurement of a hormone produced by small ovarian follicles, used to add information about ovarian reserve. I consider it within a wider preconception or fertility conversation that includes age, cycle history, and other findings. It can help inform expectations about response to fertility treatment, but it does not measure egg quality or predict whether you will conceive.

Specimen
Blood; confirm the exact specimen and collection requirements on your order.

Timing & collection
AMH can generally be measured on any cycle day; discuss recent hormone use and relevant clinical context.

AMH informs ovarian reserve and anticipated response to stimulation. It does not measure egg quality or provide a definitive prediction of natural conception.

Collect with intention.

Blood-test timing depends on the clinical question. DUTCH collection is assay-specific; vaginal swabs follow the actual kit's preparation instructions.

Keep the limits in view.

No panel measures the entirety of hormonal wellbeing, and normal results do not invalidate symptoms.

Results belong within your history, symptoms, examination, medications, and the laboratory's method. A reference interval is not a universal target for every person. There is no single validated set of 'optimal' values that applies across all assays and life stages.

Availability, ordering requirements, collection fees, and eligibility vary by test and location. Review the current Fullscript listing and your clinician's instructions before purchasing.

Good questions. Thoughtful answers.

Do I need every test in this guide?

No. Choose a test only when its result may change a useful decision, taking existing results, symptoms and your budget into account.

Does an out-of-range result identify the root cause?

Not by itself. Interpretation depends on the assay, collection, history and the pattern across findings. A result can be one clue without being the explanation.

Where can I see ordering requirements?

Open the individual Fullscript listing for current availability, specimen instructions and eligibility. A collection may include alternatives rather than tests intended to be purchased together.
